standard booth fee: $50/event unless a different event term is approved
attach: portfolio + current body-art clearance/permit, BBP training, and requested health/insurance documents

Download the PDF and send it with your portfolio, current permits, bloodborne-pathogen training, and anything else the form asks for. Good tattoos do not cancel out missing documents.
