Tattoo Consent + Intake | LoveJoy Market
do it before you arrive so your appointment stays about the tattoo.
Medical questions, consent, and signatures. Not glamorous. Still required.
Tattoo Consent + Release
It asks for your appointment details, medical history, health screening, initials on each consent statement, photo preference, and signatures. Nothing sneaky, but please actually read it.
Download the fillable PDF.Save it to your phone, tablet, or computer before you start.
Answer the questions and read the whole thing.Fill in your appointment and medical information, then initial only the statements you understand and agree to. If something is confusing, stop and ask me.
Choose your photo preference and sign.Finish the signature fields that belong to you, save the file, then close and reopen it once to make sure your answers stayed put.
Rename the file.Use: LastName_FirstName_LoveJoy_Consent.pdf. Tiny organizational miracle.
Email it before you arrive.Send the completed PDF to hello@lovejoymarket.co with subject: CONSENT | Full Name | Appointment Date.
Use the same client name you used when booking in Square.
That helps me match the form to the appointment without opening seventeen mystery attachments.
Do not delete the completed PDF until your appointment is finished.
Bring the same valid government-issued identification referenced on the form when you come to your appointment.
The PDF is not saving my answers.
Download it before you start. Fill it in with Adobe Acrobat, Apple Files or Preview, or another PDF app. Save it, close it, and reopen it to make sure your answers stayed.
I do not understand one of the provisions.
Do not guess and definitely do not initial it anyway. Email hello@lovejoymarket.co before signing so I can answer you in writing.
What about the government ID upload box?
That box came from the original Jotform. The downloadable PDF cannot upload anything there. Bring your valid government-issued ID to the appointment. Do not email sensitive identification unless I specifically ask and give you instructions.
Can I just bring the form with me?
Please finish it before you arrive so we do not spend your appointment doing paperwork. If the file is fighting you, email me before the appointment.
paperwork questions are normal. mystery paperwork is not.
